Getting Services

We collaborate closely with various local Government Programs including the Illinois Department on Aging and Illinois Department of Human Services, as well as, the Managed Care Organizations, including healthcare providers such as Humana, Aetna, Blue Cross Blue Shield, Meridian, Molina, and County Care, as part of our Community Care Program at Healthcare Plus.

This program offers eligible seniors the opportunity to receive complimentary home care services. To qualify, applicants must meet the following criteria.

  • Be aged 60 or above.

  • Hold U.S. citizenship or legal residency (with more than 5 years as legal residency is required).

  • Reside within the state of Illinois.

  • Possess non-exempt assets valued at less than $17,500 (Note: Non-exempt assets do not include a home, car, or personal furnishings).

  • Be in the process of applying for Medicaid or already have Medicaid coverage.

Upon the submission of applications, Care Coordination Units (CCUs) from the respective counties arrange appointments to assess eligibility. Once approval is granted, the CCU develops a personalized care plan and allocates a specific number of service hours to the client.

Under the Community Care Program (CCP), seniors also have the option to designate a family member as their Home Care Aide. We understand that many clients feel more comfortable with a family member, such as a daughter, son, son-in-law, or daughter-in-law, tending to their care needs. While spouses cannot be chosen as caregivers, any other family member can become a preferred worker for the client.
